Your First Visit

We want to make your first visit as welcoming and efficient as possible. Here is what to expect:

    Arrive 15 minutes early to complete any remaining paperwork

    We will perform a comprehensive exam and discuss your goals

Insurance & Payment

Out-of-Network Provider

We are out of network with all insurance plans. If your insurance pays providers directly, we will submit your claim and bill you for any remaining balance after your insurance pays. If your plan does not pay providers, payment is due at your visit and we will provide a superbill for you to submit for reimbursement. Our team is happy to help verify your coverage beforehand.

Office Policies

Appointment Cancellation

We kindly request 24 hours notice for cancellations. This allows us to offer the appointment to another patient in need.

Late Arrivals

If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure all patients receive adequate care time.

Emergency Care

Dental emergencies are seen the same day when possible. Call our office immediately if you experience severe pain or trauma.

Children & Minors

A parent or legal guardian must accompany minors for all appointments and sign consent forms for treatment.

HIPAA Privacy

Your privacy is important to us. We follow all HIPAA guidelines to protect your personal health information.
