Appointment Cancellations
Any cancellations or missed appointments without 24 hours' notice will be charged a fee.
HIPAA PRIVACY
Your privacy is important to us. We follow all HIPAA guidelines to protect your personal health information.
lATE aRRIVALS
If you arrive more than 10 minutes after your appointment start time, you may be asked to reschedule if time does not allow you to be seen.
FINANCIAL POLICY
If your insurance plan does not pay providers, payment is due at time of service - we will submit your claim and reimburse you any portion that your insurance covers.
cHILDREN AND MINORS
A parent or legal guardian must accompany all patients under age 18 for all appointments and sign consents for treatment.
