You pay an annual subscription fee to participate in the discount plan. The subscription fee covers any needed cleanings, exams, and X-rays, and 20% off all other services!

Includes 4 cleanings per year, plus all of the benefits on the standard plan!
Fees for services provided must be paid at the time of service by cash, check, or credit card (No CareCredit or other similar financing)
For the first year for patients with periodontal disease who require deep scaling and root planing (deep cleaning). Tier 3 thereafter.
